Run the check list.
Strip the gun down and look at the slide.
Take a live round and gently slide it up the breech face. Is the extractor tight enough to hold the cartridge?
Does the extractor hook stay relatively steady, or does it rotate as the case slides up the breech face? A little rotation is normal.
On the frame, is the ejector tip looking good, or does it look like it might be chipped? Is it firmly held in the frame?
Are you using the factory weight recoil spring? How about shock buffers?
As for ammo, is it factory, commercial reloads, your own reloads?
