223 Primer rim stuck in case after decapping.

Just finished up reloading some 223 for tomorrow, and had the weirdest thing happen to me. Two of my 223 cases after decapping, had the primer rim stay in the case. Only the center of the primer came out, basically looked like they had ring crimps.

These are uncrimped PMC 223 cases that are on their third or fourth reload. I couldn't get my reamer in, so I just tossed them. I'm presuming this is caused by overpressure?
 
In my experience moisture causes ringers. Range pick ups or wet tumbled bras not dried properly. I'm sure there are other causes as well but in every case I've experienced moisture was the culprit.
 
I had the same thing happen when buying bulk once fired military Winchester 9mm cases dated 2010,2011 and 2012. I thought the problem was caused by over straining/stretching the rounded edge of the primer cup causing it to separate. The primer cups were not plaited and may have bonded to the primer pocket or the primer sealer was made by Gorilla glue.

It broke my heart but these once fired cases ended up in my scrap brass bucket......sob...hic...(I have brass OCD and hate tossing brass) :(
 
Thanks to both of you for your responses, and happy to see this isn't just me. Hopefully it doesn't happen to often, I also have a very hard time disposing of brass (Although I have enough 223/5.56 brass to last a lifetime).
