.22LR inserts for 12 gauge shotguns... where do we get?

I feel very dumb asking this question - but for these inserts, I understand they work by allowing the hammer to strike the round and cause the projectile to be fired, but how do they effectively exit the barrel without causing it damage (as I expect they might bounce a bit in there) and furthermore, would there be any accuracy all all from them (not just the .22 - have seen the others for various calibers)? The extended rifled model above seems that it might solve that issue but otherwise why would anyone use these? Honest question looking for feedback as I know little about this.
 
I purchased a few from Short Lane Gunadapters.com. They used to ship to Canada, but I see it now says they do not ship internationally. I tried an inexpensive non-rifled 12Ga to .22 adapter which was pretty useless - the bullets would tumble and keyhole at even short ranges. I then upgraded to a rifled 3" 12-22 adapter which works better, but with a 3" barrel and my shotgun's bead sight, the accuracy is still not great (minute of one gallon paint can at 20 yds). The 12ga to .45ACP was even worse. The only really useful adapter, that I tried, was the 12Ga to .410 adapter. I suspect the longer adapters would be much better.
 
I have a krieghoff adapter in 22 mag that is 18 inches long and it is adjustable .I use it in my Savage 2400 12ga/308 combo and it is very accurate(of course the savage has iron sights and is set up for a scope as well).Without sights your not going to get great accuracy out them.

Anybody know where or if you can get a Krieghoff 22 adaptor in Canada? I think they need to be fitted by a smith to work optimally. Anyone ever have one done?
