.38 spl shaving copper cup edge off when loading

Ive got a quick question, every so often, loading .38spl i get one where the bullet starts to go in, sometimes gets jammed in the seating die and takes a chunk off the copper cup and a bit out the side of the bullet, am i just not setting up my lee flare die deep enough? others load in no problem, brass length is the same. Im loading 155 swc R and R bullets.
 
Not sure what you mean by "...takes a chunk off the copper cup". Do you mean the brass case?

Use more flare and make sure the bullet is seated straight in the case before seating. Lee dies are OK but I really like Hornady dies because they have a sliding bullet alignment sleeve in their seating dies which makes it just about impossible to seat a bullet crooked.
