870 fail to feed/eject

Hey all. New to shotguns! Just bought an 870 from the EE recently and finally had the chance to shoot it. Used 2 3/4 federal lead no 7. Fit 4 in the tube (it has an extension) but the fifth wouldn't fully fit in. After a shell was fired, when the slide was pulled back the hull wouldn't fly out. Just kinda sat there or stuck out. Also another round wouldn't chamber when I racked the slide, unless I pointed the shotty up and gravity pulled a shell down. I think the follower/spring may be stuck possibly on the extension. Any other ideas?
 
I bought a brand new one and the plunger behind the extractor was stuck, would eject sometimes but would usually just pull the shells out of the chamber and drop them inside the receiver. Glad I tried it out before I took it bear hunting...
 
Don't know about the hull not ejecting, bad extractor maybe? But if the shells aren't leaving the tube without the help of gravity then I think its likely the follower is getting at where the extension connects. It could also explain why you couldn't fit the extra round in even before the tube was full. I would double check that the extension and the extension connector are correctly tightened. If the problem doesn't go away after taking it apart and putting it back together, then I would take some sandpaper to the follower and round out both the front and back edges.
 
Just an update. After a full disassemble and clean I found that there was some extra metal not cleaned out of the existing tube (at 2 holes drilled there). The follower was not getting hung up in the extension but that metal. 2 min with a DREMEL and a sanding stone cleared that right up!
