Any clue what's happened to this revolver?

the very reason i weigh the WHOLE DAMNED THING before placing the round in the"finished- ready for the range" box- esp with the 44 - that final check shows sqibs and doubles,and only takes a few extra seconds
 
H110, is my favorite powder for that very reason. IMPOSSIBLE to get a double charge. Still could have been avoided by visually checking the level in every round. It can happen in any gun though, even a Ruger SuperRedhawk, pretty much looked exactly the same. It was posted here a few months ago, originally from Blueline I believe.
