So there was I dry-firing and resetting the trigger very slowly when the hammer dropped - I was able to duplicate the problem but only once by resetting the trigger extremely slowly. Fortunately I wasn't using live ammo when this happened.

My question -- has this happened to anyone else using the stock trigger components? The rifle isn't that old, with maybe 3k rounds though it, so I find it hard to believe that the components may have worn out already. I'm assuming the fix to this to order a new set of components. I was hoping you folks could weigh in on this and share your experiences with stuff like this.

Did you do a trigger job on the rifle? - Most AR parts are surface hardened only, so even 'cleaning up' the trigger can drastically shorten the life of the component.

If you go through the surface hardening by doing a trigger job........that's one hell of a 'cleaning up'.
 
That is a common problem. The disconnector "hook" wears and releases the hammer when you let the trigger out.

It could be excessive wear on either/both the trigger hook or hammer. Best method to fix is replace both. There is an actual way to repair it, but easiest to just replace.
