AR-15 Failure to extract and accuracy problems HELP! *** UPDATE POST 26***

You want to make sure the barrel is seated properly before torqueing to spec. I didn't invert the process, just find it works. Check AR15.COM for the tips and tricks
 
What do you hope to achieve by torquing repeatedly? When a fastener is to be preloaded, it is torqued once to the specified level and then left alone.

One is not supposed to torque it three times. One is supposed to hand tighten it firmly twice, then torque the third time. The process isn't really necessary but the theory is to help 'match' the threads (for lack of a better term off the top of my head). I suppose it's somewhat along the lines of lapping scope rings. In theory one shouldn't have to..........

Realistically, if it's torqued properly to spec once, it's not going to come loose on it's own.
 
The clamp on gas block could be the culprit too....they tend to leak gas as they don't usually mate up that great. If it was mine, I'd take it apart (upper), reinstall everything and torque the barrel down tighter and same with the gas block. Take it out and try it again
 
Almost every time i see someone having issues with their ARs at the range its a loose castle nut , people build a AR or install a sling attachment and dont stake that castle nut, if its loose you will have all sorts of feeding and extraction issues . Now i just keep a AR multi tool in my range bag ..
 
**UPDATE**

After sending my upper receiver assembly to S&J they determined that the barrel was defective. it just wouldnt group. They even removed it from my upper and placed it into one of their own in order to rule that out. Simon didnt have stock on any 16" S&J replacment barrels, so i was given a Daniel Defence S2W Profile 18" barrel, I just recently went to the range with it and its a VERY accurate barrel. i was hovering around 1.25 - 2" at 100 yards with Federal Red/Black box 55g ammo. I dont consider myself a marksman, so i was quite impressed with the acuracy using cheap ammo. Handloads/match quality ammo would probally be amazing in this Rifle. S&J might have taken a little time to get this issue sorted out, but they took care of me in the end.
 
That is pretty great, and not even something I would have guessed as the problem.
I have a 10.5" 1/7 S&J barrel currently and for some reason it shoots 55's way better then heavier bullets....not complaining for sure....
