AR 15 failures

Is it a complete lower - or something you assembled. I am more interested in who actually made the hammer spring and hammer - also, who made the firing pin?
 
Have you dis-assembled your bolt and checked it for wear and tare plus removed excess carbon build up?
Is it a two stage trigger?
If so are you able to ride the first stage when firing?
If at all possible, please post pic's of a dis-assembled bolt. Also close up's of firing pin plus the hammer when the trigger is pulled.
 
400 rounds (of American ammo) should not cake up the firing pin channel to cause a problem - unless there is some sticky preservative in it which was not cleaned.

I will say keep firing it more - and see if the problem become more frequent. If it happens more and more, something part is giving. I will do the firing pin first, and next hammer spring. Also, take a look and see if the hammer is deforming (not heat treated properly)
 
Two FTF's outta 400 rounds of AE and people are saying you should rip the gun apart and look for damaged parts...?

With a .5% rate of FTF I'm thinkin' you don't have any problems with your rifle. It's the ammo.

I had 1k of that ammo run without issue, and to have two that close.....
 
I'm tending to agree that it isn't an issue with the rifle itself. The fact that it racked and fired subsequent rounds says to me that there's not really anything "wrong" with important parts...

I would give everything a check over, but I wouldn't jump to the extreme of ripping apart the lower receiver over 2 FTFs...
