Just a reminder for you all that when this happens, STOP firing unless you want to risk buying a new gun, even if you are on a timed string.

It happened to someone today at the range. The round went puff instead of boom. There was a failure to eject. An obvious primer with no powder ammo. Ammunition was brand new recent manufacture Winchester brand.
Operator proceeded to chamber another round and fire it. So he got an overpressure going into the gas tube and into the action where the explosion occurred. Both bullet exited the barrel as they were not found. No injuries.

Upper broke off retaining pin

Flared magwell

Broken magazine

Exploded receiver and bolt carrier

Kaboom!

Glad he wasn't hurt. I'm kinda paranoid the same thing's going to happen if I ever get around to testing my AR. Though I guess the worst I can really expect is that I indexed the gas block incorrectly and it won't cycle.

What stood out to me was that is was Winchester Ammo. Normally a squib here and their is nothing too notable, but Their have been a few threads lately about production flaws with Winchester Ammo. Cracked cases, deformed bullets, and now a squib, and that's just from what I have read about. makes me wonder if their QC/QA has taken a nose-dive to meet demand.

Not dawning the tinfoil hat or anything, and I normally really like Winchester as well, its my go too hunting round and I found the brass to be a decent quality as well. But several incidents of flawed ammo makes me wonder.

That aside that's one hell of a failure, and good that no injuries.
