BC Q 3 and 4 Sign up

Looking for a way to sign up for the BC qualifier 3 and 4 without access to the IPSC BC website.

Is there a sign up sheet available to fill out and send in?

I cannot acccess the BC site from work.

what happened to our online form that was online fillable (is that the right word?).
why don't we use Omens online setup? seems to work well for the guys back east.
