Benelli Super Sport Malfunction

I occasionally get a malfunction with my 12 gauge Benelli Super Sport.

After a shell has been fired, extracted and ejected, the next shell from the magazine will sometimes hang up between the bolt and the forward edge of the loading port. The rim of the shell is not under the extractor but is also partly out of the loading port.

Once in a while, instead of hanging up in this fashion the new shell from the magazine will end up coming right out of the gun and be found on the ground at my feet after a click that should have gone bang.

I fired about 400 rounds yesterday and it happened once. I took a photo of it just as it was with my phone. Here's what it looks like:

You can see that there's nothing odd about shape of the shell and it has a nicely tapered mouth.

I keep this gun in spotless condition (it's dirty in the photo but that's about 300 rounds in for the day). I can't find anything amiss with the trigger group or the shell lifter. It's almost like the shell is getting pushed back out of the magazine so fast that it bounces off the stop in the trigger group before it can slide up under the extractor. I can't say if it happens less with fewer rounds left in the magazine or not (meaning I don't know if mag spring pressure has an effect). I'll have to start checking that. When we shoot informally at our pit I load the magazine with 4 rounds.

This is the only type of malfunction I ever see with this gun, every other part of the firing cycle is fine.

I'm shooting my own reloads here. I've tried increasing the amount of taper on the case mouth (that's one of the more tapered ones in the photo) and it may have made a slight difference but it still happens. These shells are 1 1/8 oz at 1300 fps so it's not being caused by a load that is too light.

This only occurs rarely but it still bugs me. I may go 300 rounds without a hitch but then may have it happen twice within 25 shots. It's random.

Any thoughts on what's causing this or on how to correct it would be appreciated.

*edit* I'll add that there are 3 other Benelli Super Sports in use with our group ( two of these are 20ga and one is 12) and I haven't seen this malfunction occur with those, although mine gets quite a bit more use. There's also a Benelli Super Black Eagle II that gets used a lot (pretty much identical action AFAIK) and haven't seen it with that either. The SBE II is run dirty, dirty, dirty and seems to keep on going until the chamber is so dirty that a round won't slide in. Anyway, this problem does not appear to be something that all Benellis do from time to time.
