Beretta Silver Lark 12g problem

I have this Beretta Silver Lark 12g 1960's era shotgun and it is basically a single shot and I don't know enough about them to figure out what is wrong. I load it up with three round and it fires the first round and ejects the casing a mile but won't pick up the next round. Any ideas??

If you want to load three rounds, buy a repeater.

Check the shell latches to make sure that they haven't been altered or tampered with.
If you were to cycle the action by hand with only 2 in the magazine, does it feed onto the lifter?

it's a semi auto not a pump.

It still has latches and you can still cycle the action by hand...
