Beretta trigger not resetting the 2nd shot

The OP's gun is a DT11. No need to take the stock off because it has a drop-out trigger. I have the same gun and it is better than most for resetting even if off the shoulder.

Well a drop out trigger group should simplify an inspection and fix.
 
Helloo Folks Update:

Cleaned the gun, with G96 and compressed air
Changed the firing pins
Went to the club yesterday and it did it again 3 times on 100 shells.
When it jammed i took out the forend and disassembled the barrel from the receiver, pulled out the trigger and the 2nd hammer was released.
I think there is only 2 scenarios here:
1) when i break my shotgun and load it up, the 2 nd hammer doesn’t get cocked
2) when i fire my forst shot something happenns and releases the second hammer as well.

Although i am more inclined to think of the first scenario.
 
Hmmm... If I understand your wording - The gun failed to fire the second barrel. You took off the fore-end, opened the action and removed the barrel. Then you pulled the trigger and it went off. My thoughts are as follows:
1) By removing the fore-end before opening the action, you are preventing the hammers from being recocked. This is typical of the Beretta 68X series, likely the same for the DT series.
2) The second barrel is therefore already cocked, and going off when you pull the trigger.

So that leaves me to conclude that you are not fully releasing the trigger between shots ????
