Usually it's a sign of a low pressure handload. There is not sufficent pressure to expand the neck of the brass to seal the chamber. Some of the soot and gasses from the charge creep between the neck and chamber.

I hate to ask ( because I'm looking not to smart ) how does one properly clean the chamber , usally I stick a rag into the camber and give a few twists.For the barrel I use Shooters Choice until the patches come clean , also read somewhere that too frequent use of the brush is harmful to the barrel.
To scrub the neck area I usually one a brush one caliber bigger than the rifle shoots. If you are worried about damaging the rifle, use a nylon bore brush. I think more damage can occur from the cleaning rod itself, use a bore guide to keep the rod centered in the barrel/chamber.
