A bullet that doesn't leave the barrel is very noticeable; it's feels like a cap gun with no recoil... if it is semi-auto, chances are the slide will not have ejected the spent brass either.
I'd also assume heavy leading (a look down the bore could confirm), or a flaw in the steel as mentioned earlier.
