I've seen three...

One was on a 22LR rifle, one on a .22LR Ruger Mark III handgun, & one on a CZ75 9mm.

The rifle is my dad's, the Ruger is mine :( and the CZ75 belonged to a shooting buddy at the time & it happened while we were shooting together. It's now mine as well.

Both handguns have been fixed, BTW! :)

we where out shooting clays after a dismal day in the field. We had some OLD Czech shells with the horse hair wads, as far as I can tell, on the first shot I had a popper and the wad got stuck?? The second shot and this happened. About an inch of the barrel was gone, I had a one inch piece stuck to my forehead and the end of the barrel was on the ground. The gunside of the barrel looked like it was machined straight.
 
I bought a Win model 1912 in 20 guage original 2&1/2 inch (1817)with a ringed barrel(about 1inch behind front site) and gave 400 for it.

Shoots great ! Cuts it's collectability in half or more (still worth what I paid for it)I fiqured some body shot steel through that nickle steel barrel at some time.

I voted for your pine needles but I still have a story to share.

Bare in mind this is hearsay but I have seen the rifle. A fellow bought himself a new .270 Featherweight Model 70. His kid decided he wanted to take it hunting so his Dad loaned him the rifle. A couple things may have happened at this point, but he was pretty sure he dipped the muzzle in some snow and it froze. He pulled the trigger and no word of a lie, the barrel split into three strands for about 15 inches, and the muzzle end of all three tore into forks about an inch. Not only that, it was almost perfectly proportional and even, save for one of the torn ends that wasn't quite the same as the other two. (I wanted to buy it because I thought I could make a really cool lamp out of it! :p) Anyways, he paniced and wen't out and bought another Model 70 Featherweight to replace it, apparantly without telling his Dad what happened. I don't know any more, but it surprised the Hell out of me to see that. There was a bulge where the muzzle end was torn, you could see it. But the rest was torn like nothing I would ever have expected. You almost think he would have to of fired twice to do that.
 
A friend's son blew the end off a Win 12 by shooting it with a plugged barrel. Another friend brought me a Win 88 with a bulged barrel near the muzzle for cutting and recrowning. I've had this happen in handguns a few times, newby shooters and misfires/squib loads each time, once with a S&W 1917, once with a S&W 29, and once with a Colt 1911A1. - dan
 
About 25 years ago I purchased very cheaply a model 27 Smith & Wesson in .357 it had two small bulges in the barrel, I went on to shoot and win competition after competition with this revolver over the next 10 years.
 
I've had it happen twice. Both times on .243's. One was mud inthe muzzle the other left an obstruction in. First onwthe gunsmigh cut off a couple of inches and recrowned. The other took a whole new barrel.
 
I had a mod 70 pf that was "ringed". I don't know when it happened but it must have been me that did it because I bought it new and nobody else used it. I suspect I got a piece of ice or mud stuck in the muzzle while riding the quad. Couldn't see a bulge from the outside but it shows as a bright ring about a 1/2 inch from the muzzle if you have a good bore light. I got it cut down an inch and it's as good as it ever was.

I bought 2 shotguns with bulged barrels - older guns with thin softer steel that did not take kindly to steel shot. If the bulge is not too bad it can be worked out but nothing guaranteed, sometimes it works and sometimes it doesn't.

Happened to my 17 Remington. No idea what caused it, accuracy just went off the map. Found a nice little ring about 2 inches in from the muzzle, no external sign at all. Cut it off and was back in business.
