Can someone explain this misfire with factory CCI?

Had this misfire at the range today, 9mm CCI 147gr Blazer Brass. Just curious what happened, assuming its bullet issue not a gun issue. Just clicked then sizzles with smoke out the chamber. Improperly set primer?

I’ve had a Barnaul round with an improperly set primer and same thing, click and a puff of smoke.

I’ve also had a Norinco 7.62x25 case with no flash hole. That jammed the gun up pretty good but basically sounded like a dud and there was no smoke.
 
I recently made my way through pulling tens of thousands of rounds of range duds. That was definitely one of the more common defects. I saw it across brands of all kinds including obvious reloads.
 
Too clean an edge. Perhaps the primer cap was punched from too close to the edge of a sheet of metal and was missing that much, and got past inspection?

Looks like the charge burned out through the flash hole and didn't get the bullet moving.
 
Do you kind of look at every round as your loading it into the mag ? especially nowadays when things are in such short supply . QC has gone down hill somewhat on quite a few products. do yourself a favor get a bullet puller and pull the bullet out of that case and see if it has a flash hole in the bottom of the case. if not then let the store know that you bought the ammo from and I'd shoot off an email and photos to CCI . next time you load a mad or any gun give our ammo the once over before loading it. you got off lucky as in no hot gasses bled off your way .
