Yes to both your questions.

We do keep the credit on file but you must request we apply it. In the past, we had applied credits automatically when the customer would have rather saved the credit. To avoid upsetting customers wanting to save their credits, we ask that you request we apply the credit in the customer notes section of the checkout and refer to the previous order number. The credit for this campaign will expire January 1, 2016.

No.. The campaign applies to purchases made from October 15th to November 15th.

I was just shafted to the tune of a $25 credit. No where does it say that if you apply a store credit to your order, this credit is subtracted from the total amount of the order.
Example: My order was set up for $500. I applied a credit for $100 and thought I would get a credit for $50. Wrong. The credit amount is for the balance of $400.
Wish I had known this. Thought other people might appreciate knowing this.
