Cooey 60 magazine follower falling out?

I was out looking for a couple bunnies and having a nice time in the woods. All of a sudden there he was, standing tall. Worked the bolt and then, huh? The magazine tube (follower) fell out and my rounds ended up all over the place in wet leaves. I put it back in and worked the bolt a bunch of times and after a few tries it popped out again. A zipper tie kept it in place for the rest of the trip but I need to figure out how to fix it. Everything looks normal, worked perfect for years before this. Nothing happened to it, wasn't lent out or anything. Anyone have any ideas?
 
The inner magazine sleeve is anchored by a latch pin at the muzzle end of the external magazine tube - is that pin still there? It could have broken off, worn off or fell off? Is the external magazine tube still round, that the pin can latch to it??

The part called the "follower" and its long coil spring, are normally captive inside that inner magazine sleeve.
 
Did the tube fall out forward? or all the ammo and follower come out the receiver?
It it fell out the front , that is missing pin or notch buggered
Out the back, receiver, than the tube needs to be restacked , I made a thingmagig out of a split washer, small vice and small hammer.
small center punch can help.
easy to bugger, miss shape if not careful.
 
marshall - good idea if that crimp on the inner mag tube needs to be established or re-established. I guess that I have not been around long enough - have seen multiple inner tubes fall out forward - Cooey 60, Win 9422 - but never seen shells come spraying / falling out at the receiver end - I can sure see how that could occur, though!!!
 
With mine , the follower pulled out when I was cleaning it .
The end of the tube has stretched out , allowing the ball end of the follower to pass through.
 
The shell lifter rides on a cross pin, if the pin fell out, the outer mag tube could loosen, spilling the rounds. The follower would be behind all the loaded rounds in the inner mag tube.
