Not worried about tossing a few empty .45acp cases but just trying to learn here just so I know what is acceptable and what is not. Being new to the reloading game, there seems to be many questions as I encounter new issues. Should I toss these cases. Are they at the point that failure may occur if reused.

Go ahead and use them. Then you'll know for sure what can be reused.
To me they are all acceptable.
Were they all fired by you?
 
OP,
With out specifics of whether said cases were factory or reloads & the platform in which they were fired.

The dented cases will re-size & fire form on the next firing.

To eliminate the dings you have 2 options-
Blend out the point in the ejection port that is causing the ding.
Install a couple pound heavier recoil spring if fired in a semi auto platform.

The case with the mouth issue is the result of a feeding issue.
Use that case to build up a dummy round to plunk & cycle test w/ your projectile of choice.
 
If I found these during the sorting process, they would get thrown out immediately.
Why waste time cleaning them only to have them jam up the brass feed tube or not seat on the shell plate, this makes no sense.
If you throw them out immediately you save time and the cost of a primer, powder and projectile if they later fail the plop or chamber checker.
I chamber check 100 reloads at a time and I still find the 1 or 2 that don't make the cut under the best conditions.
My dad used to say "no matter how hard you try, you'll never turn chicken #### into chicken salad."
