Thanks for the update. I've not mine to the range much, but was having some similar issues which I decided must be ammo specific but haven't had a chance to try others since. Ill keep this fix in mind. And thanks to the guy who gave us the solution.I removed the left extractor (punch pin from below bolt) and went to the range today to test. 100% success rate on a bulk box of Federal Automatch. I was previously running at ~8% failure rate on this and more expensive ammo.

Since removing the left extractor I went ~1,200 rounds without a single issue.Can anyone confirm if this problem is fixed after removing extractor?
I sent mine for repair and it still got the same problem.
Hold on, you can put the left side extractor on the right side in case it breaks? Sorry I don’t own the gun but just wanna knowHad the same problem, tried the same fixes. Decided that 2 extractors is one too many. Removed the one on the left side. No more problems. Plus now it's more aerodynamic and I have free spare parts.
Weird issue though, no signs of wear or peening, no carbon ring, just empty cases getting munched by the bolt, while it desperately tried to chamber the next round.
Edit: Working hypothesis is uneven spring wear causing uneven extraction. Ran flawless through a Speed Steel match.
