Double feed?

Just curious as to what causes a double feed in a pistol? Is it a mechanical issue or is it caused by the magazines? I've had a couple recently and trying to narrow it down.
 
Magazine lips getting a little bent out of shape maybe? That would the first thing I would look at. I've seen failure to extract spent cases and feeding a live one into the spent one with a CZ-75 I had, but that was worn brass.

Yup, damaged magazine or brass that has seen too many reloads could result in double feeds. Check your cartridge rims, if they are badly scored with ejector marks, avoid using them for competition or serious work.
 
Nearly always the mags. The lips get opened a tick. Easily fixed on most steel mags with fine, long, needle nosed pliers.
