All muzzle breaks are loud (especially the ones directed backward). They aren't designed to suppress the flash or sound but rather redirect it in effect to reduce recoil and allow for better accuracy.
I honestly don't see the point of having one of those on a VZ-58, but s**t, people put them on their ARs... so who am I to say...

It was so loud that it was causing me to jerk the trigger and flinch. So I went back to the standard military issue one and been loving it since.
I use one (the Czech issue type) simply because the fixed stock on my 858 consistently hammers the point of my cheekbone making shooting it unpleasant after a few dozen rounds. This only happens with an 858. Just a combination of poor ergonomics vs facial structure. The brake eliminates the problem for me.
I have a 3 baffle quick discharge brake which is loud, and the standard issue one. I'd have to say the standard one I like more, it's quieter, and still stops the muzzle climb sufficiently.
