Was at the range recently. Shooting a M&P 9mm. On clearing the pistol to eject the live round, this happened.



Never in my 30 years of shooting have I had this happen. The round was not crimped properly I guess. This was factory AE ammo from the Federal rebate and sale at Xmas last year. Maybe that’s why there was a rebate? LOL!

Update.

Same M&P. Just practicing mag changes. Loaded mag but no chambering. Just mag in, mag out, mag in, mag out. And the same thing happened again! Same AE ammo. When I dropped the mag, the casing is still in the mag but the bullet fell out, alone with the powder which dumped into the mag body. The mags never hit the ground, only into my hands.

Never seen this before and now twice with the same lot and brand of AE 9mm and same gun. Coincidence?
 
I'd guess it would be a lot more likely to happen multiple times in the same lot vs over different lots. When you mass produce something in such huge quantities, errors tend to be repeated over many pieces before someone notices.
 
If this si the first time in 30 years youve seen this, I wouldnt worry about it being a trend.

Dont shoot anymore from the box, email them. They may want whatever you have left for testing, in which case they will pay your shipping expenses, and probably provide you with a refund or reimbursement for them.

Ok, I’m beginning to wonder if it’s a problem with the M&P.
Happened again! This time with a CRS reloaded nickel plated round. Mag change practice. Eject mag, no chambering and bullet falls out with casing still in mag! Exact same thing as 2 days ago!! But different ammo this time! No cycling of slide, did not drop mag on ground! 3 times with the same M&P.

What does A/E AKA Federal have to say about this anomaly?
The guy who is the Federal Rep resides in Maple Ridge and is also the Rep for RCBS iirc.
I see now there might be a firearm related issue instead of the ammo.

While this happened with two different brands is puzzleing I still can’t understand how this is a fault with the gun. 9mm crimps are fairly tight and NOT east to pull apart let alone fall apart.
 
There is probably something prying on the bullet from the slide. Look up inside and see if anything would come in contact with the loaded bullet. The top bullet is compressed on a full mag. Might even have some prying done on the lip of the mag.
 
Also OP you shouldn’t be pulling live rounds into your pistol for mag drills in your home. I would be happy to make up some dummy rounds for you to practice with. Send me a PM if you are interested.

Haha! It wasn’t at home! LOL! I was doing it at the Range with my eyes shut. Ya know? All tactical and all that! I have dummy rounds for dry fire. Thanks for the kind offer though!

Mag looks fine. The slide block looks smooth with no burrs. Due to the tightness of the M&P mags, there were only 9 rounds so the mag seated easily. It’s a mystery!

You’re telling me! Like I said, I’ve never seen this once in 30 yrs. To have it happen 3 times, with the same gun, in a month is puzzling for sure! I mean, the casing is still in the mag! And no cycling the slide. I do bang the mags in hard though!
