Out of millions of rounds made a day, there is bound to be the occasional screw up, and if it is in the setup of the machine, it is likely there will be a few in a row till they catch it and make adjustments. That is one of the downfalls of mass production.

True enough but this happened to 2 different brands. 2 were AE, the other was CRS commercial reload.
And I can’t figure out how inserting and ejecting a mag into your hand, without cycling the slide will cause a bullet to fall out of the casing which is still in the mag.
