No.

Just leave those sections blank.

I called batfe a while back.. New form isn't yet published so they say just keep using the old one.
I faxed mine down and they emailed it back in 3 days...
As for fillable forms... There are several web based tools out there Thst let you fill in any PDF.
The one I used was free and worked great...
 
I sent mine in 2 weeks ago and got it back in 5 days, I keep asking to email it but they just keep faxing it back.......
 
Faxed down -> mailed back - about 6wks turnaround - still not bad as as this my first time...

Saved a copy of ATF- F6NIA on my hard drive - could email it to you if you wanted - but it is the older one without the email option
 
Called them to see if I could email, they said it must be faxed. So i faxed down with a request to email it back, so they fax it back in 1 week. Oh well, it is readable.

'Reloads' or 'Handloaded/custom' would be fine.

Remember - you can take less than you state on your form, but not a single round MORE. I think I listed about 1000 rounds per calibre last time I went down, even though I brought less than 500 of each when the time came. You don't want to get approved for 200 and then realize you'd like to take 500!

Well... I reload my ammo so, I've always just put my own name/address.

That said... Not once has DHS/USBP ever asked about ammo. They sometimes confirm firearms, but I've never been asked about how much or what kind of ammo I have.
Like others have said... Don't be afraid to over estimate on the ammo. If I add up all the ammo on my F6NIA it comes to 10000 rounds total..

Just received my approved form6 this morning!! It took a total of 6 days from the time I faxed it off. Can't wait to shoot some matches state side. Just need to get my bb and I'm set.
 
if by BB you mean black badge, you don't need it for the US. That's just for Canada.
Its nice to have as you learn quite a bit about safety and IPSC style competition, but USPSA or 3 gun matches don't care.

Thats odd. I keep hearing that no black badge is needed to shoot uspsa (ipsc) or 3gun but I when I received my invitation letter, the person I was corresponding with specifically said that I will need it to shoot uspsa and multigun. Maybe its a club specific thing? I don't.

I plan on getting my bb to shoot ipsc anyways so not a big deal.
