Peel back some layers of the crush washer.
Add more layers or another crush washer.
Use blue locktite and the liquid in the threads tightens it and will help it snug up without even letting the locker dry fully. I did this when putting a A2 on a 16" HBAR armalite upper that no matter how hard i tried I could not index it perfectly. So I just put two drops at 12 and 6 and that helped snug it up.

If it is a crush washer, once it is flattened out you need a new one. When you use a new one and you reach "torque" you stop when it is indexed properly. If you keep going, you will just ruin another crush washer.
On the other hand "peel" washers can be adjusted for thickness (and therefore index postion) by peeling layers of them away.
