I've never gotten one from my M&P on my head in the thousands of rounds I've put through and I've never needed to change springs (no matter how weak the ammo I put through it is), but I used to get this issue with my old CZ Phantom... Apparently you can have a gunsmith slightly adjust the ejector (rather, the angle the tip of the ejector) so as to adjust the flight path of the ejected casings.
