Got an email.
Guy was improperly holding S&W 460V with support hand thumb close to front of cylinder and took top part of his thumb off.
Haven't found link to official story but it is on AR-15 forum.
I guess some don't know that ANY revolver can have high pressure gases escaping from front of cylinder.
