Hammer down Safety On?

Generally, the 1911 doesn't allow the safety to be set with the hammer down. The choices are:
Condition 1 - Cocked and locked (flip the safety to fire)
Condition 2 - One up the pipe - hammer down - safety off (#### the hammer to fire)
Condition 3 - Chamber empty - hammer down - safety off (rack the slide to fire)

Para-Ordnance has the LDA.
Cylinder & Slide sells a kit that will allow any 1911 to do this.

Otherwise, yeah, such a condition is a failure and a problem requiring the services of a qualified pistolsmith.

Normally the safety will not engage when the hammer is down.

Apparently some PDs back in the day had "hammer down" as a requirement for carry, so some enterprising 'smiths came out with options for this (and Star of course). - dan
