That looks like a Parker Hale conversion, using a two piece Parker hale mount. Front base is secured by the two top screws. The side hole is the gas vent hole, not a mount. The rear base replaces the rear sight ass'y, so there are no visible mounting holes.
You will have difficulty finding them, as they are no longer made. You may be able to use a Weaver T-01, if the front holes are compatible. A third hole would have to be drilled in the rear receiver bridge.

The Parker Hale conversions for the No4 used A20 for the front and A21 with the ghost sight or battle apperature on the rear. You removed the rear sight and installed A21 with the existing hardware. The application used high rings. The bases were also used on the Enforcer and as such don't come cheap. Collector's source are selling these for $60.00 each plus taxes plus delivery. The high rings are tough to find and will cost $100.00. Bases and rings still rerquire fitting so not inexpensive.

An easier route and an inexpensive one, is to go S&K at about $90.00. Uses Weaver style rings and is a no gunsmithing mount.

303 Scope Mounts

There are plans on 303british.com on how to make one from materials available at any Home Hardware, Home Depot, Rona, etc. It might be easier with existing drilled holes.
