Help identifying a shotgun accessory

I was perusing the Tactical Shotguns Picture thread and kept seeing this pop up.

The rail on the top and the gizmo that allows AR stocks and Grips to be attached to. What is the name of this product and where can one be sourced?

S&J hardware. Not in production at the moment according to the site, but I'm sure it won't be long for a new batch.
