stock CZ SP01 shadow. Reloads ranging from 3.8 to 4.4 grains of Tight Group. Guns are clean. Maybe too much lube? What is really surprising is that I never had issues before, and now it is on 2 identical CZ's with 6 different mags. The slide stays open every time when manually racked with an empty mag. Suggestions?

Have you tried factory loads? That would be the first test to rule out reloading variables. If it does lock back with factory loads then it is that your reloads are too light. Have you used the same load data before? You didn't specify the bullet weight if its a 124 gr bullet then you're in the right ballpark. If you have used this load data before then I'd pull some bullets and measure the powder charge to confirm the measurements are correct. Might also be worth checking the scale or calibrating it. My franklin scale "drifted" on me once and had way under charged loads.

Check your powder dispenser. @ 124 gr bullet, 3.8-4.4 gr Titegroup is a little on the high side (hot load). I use 3.6 for 125 gr, 3.2-3.4 gr for 135 gr and 3.0-3.4 for 147 gr. without any issue in both my 75 SP01 Shadowline and 85 Combat. But at one point, I had some odd heavier than usual recoil. And the culprit: Powder dispenser.

What does your grip look like?

Does your support hand thumb (assuming that you are right handed) contact the slide-stop? Try shooting strong hand only to see if the slide fails to lock back. This would eliminate the ammunition variable.

Thanks for the responses guys. I cleaned all my mags, checked my scale, cleaned my gun again....and found out my thumb was putting just the slightest pressure on the slide release.
I've confirmed all the loads from 3.8 to 4.4 now work great! Well I'm glad the problem was me and not the gun or ammo LOL.
