while I am onside with calling these "incidents" user error I believe that the training aspect has to be addressed as well, and while these holsters may work well for some Military/LEO's who require a user actuated retention device frankly for civilians I don't see the allure of them, an open top kydex holster from most any manufacturer will hold the pistol enough to prevent accidental dropping.
for leo/.mil that need a jump rated/grab resistant holster there are better choices, maybe just class the holsters as "duty gear" and allow LEO/Mil to compete with them.... and take them out of the hands of newbies..... we have all seen someone yank thier pants up while using a serpa due to not hitting the release, and yet I can't remember the last time seeing the same for any other holster.
