Hornady Die problem

Has anyone ever had this problem?
I was loading .45 Auto shells. Using a Dillon 550B and Hornady New Dimension dies.
About half way through the little snap ring on the seater/crimper (I am only crimping with it, seater plug removed) came out and wedged between the bullet/case of the round and the crimping sleeve.
This put a very deep gash in the bullet and case. It didn't cut through the case, but it is now garbage none the less...

Has this happened to anyone else? Was I doing something wrong? Or is there a problem with the dies?
I have loaded about 8000 rounds with them to date.
