I have had this happen with my home loads in this gun with those plated bullets I think they are slippery and may not build up enough back pressure I have never had this happen with cast bullets so I switched to Trail boss and a good crimp and it solved the problem

Had the same issue with several rounds in a box of Fiocchi .455 Webley in the past, I have difficulty trusting the stuff since. However there aren't many alternatives for some calibres.

I've never had any bad luck shooting Fiocchi brand ammunition prior to this happening with my 1892. My S&W 622 has never had any FTF or FTE and digests the .22LR with no problems after thousands of rounds have gone through it. Has problems with other ammo but just loves the Fiocchi stuff.

Open the flap of the box and there will be a series of numbers printed on it. This will be what you're looking for.
