ID challenge (need help): What is this "Cavalier" single shot shotgun???

Hi folks,

A friend of mine got this shotgun which belonged to his grandfather. Only info on it is: ' Cavalier"A" ' on the action, "16ga choke" on the barrel and a serial number on the barrel too.

The goal here is to figure out what this is so we can attempt to find a replacement for the long-lost forend...

Probably made by Crescent Arms, IJ, etc, for a hardware distributor, using that name.
Somewhere in my junk collection, I have/had, a forend from this sort of gun. Will look, see if I still have it.
