She pulled the trigger twice, not particularily complicated. Shooting a gun, bigger than she's used to (if she's used to any gun at all). Likely recoil caused her to release, the trigger, then she pulled it again. There's a piece of the ceiling, on display. at my range, from a guy who did exactly the same thing with a casull 454, which is the sole reason you won't find one in my safe right now (they banned anything more powerful than 45 colt or 44 magnum, after "the incident").
Embarrassingly, I have to confess that I did the same thing a few times shortly after I first picked up my 460 XVR. It was all operator error. I was hanging onto the gun tightly, just not tightly enough for a gun of that size. I wouldn't exactly call it "limp wristing", but a gun with that much recoil has to be treated as such to avoid firing "doubles". What happens is firstly the force of the shot forces the gun into the hand of the shooter. The resistance met by the hand of the shooter then stops the gun from moving further back (if you're not hanging on tightly enough) while simultaneously forcing the hand of the shooter back. As the hand goes back and the gun stays where it is, the trigger is pulled a second time, resulting in just what you see in that video. It all happened to me very quickly, so quickly that at first I wasn't quite sure of whether I'd shot a "double" or not. It was also very embarrassing. I've since learned to hang onto my big bore guns as tightly as necessary to not let that one ever happen again.

I've seen this happen with a pistol. She fired 4 rounds in succession with the pistol with the last two ripping into the ceiling.
As a personal rule with when I let someone shoot my .454 Casull for the first time and they have never shot a big bore revolver before I only let them chamber 1 round first to avoid this from happening.
What I think happens in the video is her grip was weak on the gun so when she touched off the first round the gun tried to escape her grip as her hand opened up causing the trigger to reset. Her instinct took over and she clamped down on the gun causing her to pull the trigger the second time.

I've done it myself with 9mm CZ...(waiting for the laughter to subside)...F'ing thing had a trigger job done to it and buddy who owned it thought it would be funny. It wasn't. 2 in very quick succession. I can tell you, a lot goes through your brain very quickly when the gun your holding "goes off" unexpectedly. After that experience i swore that I would never steer away from factory set poundage on a trigger. Smooth the pull? Absolutely, but never mess with the poundage.
