I just pulled my AR out of the safe and noticed that the stock was rotated slightly off-center. (I have no idea how this happened. It's always been handled with care).

So I took the castle nut off and noticed the end plate had worn the threads on the outside of the indexing channel. Thus it allows the tube to rotate. Do I need to replace the buffer tube? Or can I just torque the Castle Nut a bit harder to overcome the rotation?

Replace the tube, it will likely keep wearing. Castle-nut might have been just a bit loose which allowed enough play to start wearing it out.
best bet is get a new tube and end plate (of your choice, qd, asap etc) and torque the nut to spec, (40 +- 2 inch pounds) and stake it. Will not come loose after that. Over tightening the castle nut can stretch the AL threads on the tube and receiver. (specs available in pdf- ARMY TM 9-1005-319-23&P
