look at your normal revolver sequence you will need an octagonal or some such configuration in the cylinder to work think about it.

It does work, rim fire ammo in a large diameter cylinder with appropriately spaced barrels equals functioning revolver.

But they DO line up. You're over thinking it. The big difference is rather than the shot being fired being in the 12-oclock position at the top-most point, they would be more like at 11 and 1 oclock, with the 12 oclock position being between two rounds.
 
^^^ yep, it would work just like a regular revolver. You can do that in .22 because the bullets are a small diameter. If you tried to make a similar revolver in say, .357 it would end up being a very wide gun, because of the spacing required between the barrels.
