Ithaca M37 FTF on 3rd round during slamfire

My last two trips to the range I've had a FTF when slamfiring my 70s Ithaca M37. It feeds and functions fine when recambering and pulling trigger. Anyone have any ideas and what the issue could be?
 
Is that "failure to fire" or "failure to feed"?

If it's a failure to fire, perhaps you are not racking the slide all the way and the sear is not catching to #### the hammer/striker fully.

Failure to feed I have no idea.

sorry for not clarifying, its a failure to fire. The round chambers just doesn't fire, I then eject that round and it continues firing as normal. I could be short stroking on the 3rd round every time, some kind of weird muscle memory thing. I'll have to pay attention to how I'm stroking :)D) a little better next trip to the range.
