Lee decaping die problem

So I have a new lee decaping/sizing die but no matter how much I torque on the nut to hold the pin in place after 3-4 cases the pin gets pushed out the top of the die and it won't deprime the case anymore. I set the die up so the bottom of the die touches the shell holder as the instructions say but.......
Anyone had this problem? How do you fix it?
 
Had a simallar problem with 300wm nickle brass, found there was a bit to much case lube built up inside the die, cleaned it up nice and dry then, tightend up and worked great. One 308 norma die had a similar problem, we ended up haveing to replace the decaping pin, this was on a rcbs die on both occations thought, I have never had a problem with Lee dies.
 
which caliber dies? I was decapping some military 9mm cases and my pin fell right out. turns out the flash hole was smaller then normal and the pin was getting stuck in the smaller flash hole and pulled out
