The rail mounts to the adapter with two hex bolts and to the mag clamp. Solid and no play what so ever.

What's recommended? The S&J rail system or the Mesa one.

I have no experience with S&J besides the tube extension. As with other's experience, S&J was a little lacking with communication and shipping times, but I'm really happy with their quality. Definitely worth the wait in my opinion. The biggest deciding factor was the mounting style of the mesa rail and rail design with the heat shield. I know it's more of a cosmetic thing but that's what tipped the scales. Although I was warned away from S&J from a few respected people but I think those were more for personal reasons. But if their adapter kit is the quality of their tube extension, I think you'd be good with either one.
