Yup 304-616-4551 is the fax #.

You can used the faxed version if it is clear enough to read the numbers. They will mail you a copy if you call them and ask them but that takes another couple of weeks or more. I always use the fax version.

can hardly read the fax copy. so i guess i need to call and get a copy from them. don't want to be hassled by "THE MAN" at the border.

I have faxed mine last week but didn't give them return fax nr to receive it in mail. This is first time I am applying, hope to get it.

Yes. That is the fax number. I see that they have another fax number, the "NIA Fax number".
