Seen that on homemade slugs when the maker skimps on powder load. Primer ignites and just spits powder out with the slug if it actually gets to the end of the barrel. Ive had it happen with a badly crimped blank, partial ignition rather than a full one.
Seems that they were working so hard on terminal balistics that they forgot about internal and external.
