M-14 op-rod problem, Help!!

Hi all, my friend's m14s has an interesting and yet very distrubing habit, the op-rod jumps out of its track in the reciever when being fired. I have two m-14's and have never had such a thing happen and i have an older norc with at least 4000 rounds down the tube. My friend has only fired 60 or so rounds out of his.

Does anyone have an idea of what could be done to fix the problem?

The barrel could be out of index putting sideways presure on the op rod.

The op rod could also have been fitted incorrectly and could be too loose. My can-am norc shorty had an op rod that was so loose i could almost pull it out of the track.

So you could have your barrel checked for index and try replacing the op rod. I'd also check if the op rod guide is on more or less correctly.
