M305 Bolt roller question

Hello all.
I was wondering if anybody has came across this issue. Today I took my new to me M305 out for the first time. I had a stoppage during firing that I cant find any information on.

On my second shot, the gun fired but failed to extract the casing. The bolt was in the forward position and sitting just upward of being fully in battery. The bolt roller had come out of the charging handle. I could operate the charging handle but of course the bolt remained still. I disassembled, added a bit of oil(maybe a little dry), reassembled and shot 10 more rounds symptom free. After reassembled I looked for any indication of what might have happened. The charging handle has no real side play and it looks to me that it would impossible for that roller to come out of its hole.

Now I know it worked flawlessly 10 times but I would like to know if its operator error or a rifle issue. I am a little unsure of reliability as this was only my first time out.

The old owner shot it very little but said he had no issues. Everything seemed good when disassembled. When I manually work the charging handle everything feels tight. I pushed and pulled while working the action and can not see how it could pop out. But it sure did. How would I check it for a loose oprod guide besides by feel.

It happens from time to time, nothing new for this platform.

Things to look for:
- as mentioned check op rod guide
- check for bent op rod

Look for the lube threads, these guns need to be greased. And dont forget to lube the bolt roller and the slot it goes into.
 
I've seen some people recommend a .45 ACP casing as a bolt roller greasing tool. I've found that a cut down .44 magnum casing is an almost perfect fit. Yomamma has some pretty slick looking little units as well.
