If that rifle was re-barreled, it could be a chamber issue. If the chamber was cut, say, 50 thou too shallow, the "fix' could be the barrel shoulder being cut 50 thou too far forward.
The result would be the chambered round sticking out the chamber 50 thou more than intended. Headspace would measure perfect.
