I just wanted to share want happened to me this week when I tested my dad's new .308WIN to .32ACP adaptor prototype made for him by a local machinist.
Since the adaptor could not be feeded in the chamber from the mag, I would insert the adaptor first then put the .32 cartridge in the adaptor. At one time I let the bolt go forward at full speed and "BANG!" Well, more of a "bang" but nonetheless the rifle did fired. I knew this could happen but I just didn't think of it at the moment.
By the way, don't worry, the round just went in the ground toward the target.
